· Insurance All major insurance plans are accepted. However it is your responsibility to check whether Dr. Lewis is listed with your particular insurance company. Also please check with your insurance company about their plan of reimbursement if Dr. Lewis is an out of network provider. If you have any queries about your insurance coverage first call either Susan Spatz or Lynn Boshoff at the Thiensville office: 262-242-3369. · Billing An off site billing service handles all matters of patient billing and collections. The name of the service is FIMED and the phone number for contact and any billing related questions is 414.258-9511. Any unresolved billing issues will be addressed by Dr. Lewis as the Medical Director of Thiensville Family Health Care. All acupuncture treatments are fee-for -service. Although there are some insurance companies which may offer acupuncture as a covered service, the office policy is payment at the time of service. However a completed billing document will be provided on request and the patient may submit the face sheet, complete with the correct billing codes, to the insurance company for reimbursement. The only exception to this policy are workers compensation cases which are evaluated on an individual basis. Review the fees for Medical acupuncture. |
